RHCs and Practice Transformation

How do Regional Health Connectors support practices that are participating in transformation efforts?

 

Transformation efforts support practices in improving quality of care while enhancing the opportunity to succeed in new payment models. Practices participating in transformation efforts receive support from practice facilitators (PFs or coaches), clinical health information technology advisors (CHITAs) and Regional Health Connectors. Unlike RHCs, PFs and CHITAs are matched with practices participating in transformation efforts. All three work together to determine how and when to integrate RHC support into a practice improvement plan.

Working in Collaboration

RHCs connect with practices, stakeholders and community-based organizations to assess local priorities and determine what efforts exist that can be complemented by either incorporating practices into existing projects or through connecting community-based organizations to practices for the purpose of enhancing goals already set by the practices.
